Help me cancel my subscription

Hello, I am leaving the Netherlands and would like to cancel my mobile contract. However I cannot find the option to do this on the website. How can I cancel the contract?

I've looked into your account, and I see that my colleague Barryl already requested the cancellation of your subscription per the 31st of October.
